One-On-One Health Coaching
To set up your first appointment:
1. Please fill out this Health History Questionnaire.
2- Please fill out this Food Journal for a minimum of 3 days (5-7 is even better!) I recommend doing it as you go, each day, as it will be fresh in your head. Please record if you notice any changes in digestion, energy, mood, or how you feel, after eating something.
Please email the food journal back to [email protected] ideally 24 hours before our appointment, so I can review.
