To set up your Clear Skin Coaching appointment follow the steps below:
- Fill out this Health History Questionnaire.
- Fill out this Food Journal for a minimum of 3 days (5-7 is even better!) I recommend doing it as you go, each day, as it will be fresh in your head. Please record if you notice any changes in digestion, energy, mood, or how you feel, after eating something. If it’s easier, you can keep a running note on your phone instead. Please email the food journal back to [email protected] ideally 24 hours before our appointment, so it can be reviewed and you can get the most out of your session.
